OF SPECIMENS AND RADIOLOGY FILMS

For
specimen shipping, the site should:
|
|
|
Follow
the specific protocol schedule and instructions for packaging and shipment
of specimens. |
|
|
Send the specimens directly to the person and address specified in the protocol.
To avoid delivery delays, ship specimens on Mondays, Tuesdays, or Wednesdays
only. If specimens must be sent on another day, the site that is to receive
the specimens should be contacted to ensure that a contact person is available
to receive the shipment. |

|
Attach
a copy of the protocol-specific transmittal form (e.g., Pathology Submission
Form) available from the protocol specific materials section of the member's
side of the website provided by the Cooperative Group. |
|
|
Attach
copies of requested written reports (such as pathology and x-ray reports). |
|
|
Retain
copies of written reports that were sent, including transmittal form and
shipping receipt. |
|
|
Follow the protocol-specific instructions for submitting a copy of the transmittal
form to the CTSU Data Operations Center. The CTSU will retain a copy of
the transmittal form. |
